Academic Misconduct
Use the report link below to contact the Dean of Students Office with questions related to University of Wisconsin Systems policy Chapter 14 on academic misconduct, or to submit a formal academic misconduct report. Academic misconduct violations include:
- Seeking to claim credit for the work or efforts of another without authorization or citation
- Using unauthorized materials or fabricated data in any academic exercise
- Forging or falsifying academic documents or records
- Intentionally impeding or damaging the academic work of others
- Engaging in conduct aimed at making false representation of a student’s academic performance
- Assisting other students in any of these acts
Academic Struggles
Use Navigate to document students’ academic struggles and interventions you have used to support the student. Academic struggles can include:
- Poor class attendance
- Missing assignments, tests, quizzes, etc.
- Declining performance
- Lack of classroom participation
- Disrupting classroom learning
Behavioral Misconduct
Use the report link below to contact the Dean of Students Office to report misconduct that violates University of Wisconsin Systems policy Chapter 17 and 18. Behavioral misconduct may include:
- Dangerous conduct
- Harassment
- Hazing
- Illegal use, possession, manufacture, or distribution of alcohol or controlled substances
- Disruption of university-authorized activities
- Forgery or falsification
- Misuse of computing resources
- Violation of criminal law
- Serious and repeated violations of municipal law

Bias Incidents
Use this Bias Incident Reporting Form to report discrimination, harassment, or retaliation based on a protected classification. Protected identities can include but are not limited to:

Child Abuse and Neglect
UW-Platteville employees and volunteers must report suspected cases of child abuse and/or neglect to the UW-Platteville Police Department in person or by phone at 608.342.1584. Child abuse and or neglect may include:
- Physical injury
- Sexual intercourse or sexual contact
- Sexual exploitation
- Permitting, allowing, or encouraging child prostitution
- Causing a child to view or listen to sexual activity
- Exposing genitals to a child or exposing a child’s genitals
- Manufacturing methamphetamine with a child present
- Emotional damage that has been neglected
- Refusal or inability to provide necessary care, food, clothing, medical or dental care, or shelter
Clery Crimes
If you are a Campus Security Authority, use the Campus Crime Reporting Form to relay reports of Clery crimes that have occurred within the main campus Clery Geography. Clery crimes include the following:
- Aggravated assault
- Arson
- Burglary
- Dating violence
- Domestic violence
- Fondling
- Incest
- Motor vehicle theft
- Murder and non-negligent manslaughter
- Negligent manslaughter

Sexual Assault
Wisconsin State Statue 36.11(22) requires all University of Wisconsin-Platteville employees who have witnessed a sexual assault on campus or received a report from a student that the student has been sexual assaulted, shall report this to the Dean of Students Office. Sexual Misconduct
If you are designated as a responsible employee, use this Sexual Violence and Sexual Harassment Reporting Form to report sexual misconduct that includes students, faculty, or staff members. This may include the following:

Threat to Self or Others
Use this Behavioral Review and Recommendation Team (BRRT) report form to report student, faculty, or staff behavioral concerns that include but are not limited to the following:
- Hostile, aggressive, or disruptive behavior
- Talking or writing about suicide or homicide
- Evidence of self-harming behaviors
- Inability to communicate clearly
- Disjointed thoughts, garbled or slurred speech, loss of contact with reality
- Unexplained crying, irritability, anxiety, or anger
- Beliefs or actions at odds with reality; seeing or hearing things that are not there
- Significant lowering of performance
- Changes in attendance or performance
- Decreased ability to concentrate
- Changes in personal hygiene, dress, or appetite
- Comments idolizing school shooters, etc.
